You should complete this form if you want to make changes in dependent coverage during the Open Enrollment Period (January) or if you are newly eligible for coverage and have been asked to complete an application, but do not have an application.
For a Lincoln Life insurance Beneficiary Designation form,

You should complete this form if at any time you wish to change your beneficiary, or if you are newly eligible for coverage and have been asked to complete this form.
For an election form for Waco's Cafeteria (tax savings) Plan,

We recommend that you complete this form if you have health insurance at Waco and have dependent coverage for which you have payroll deductions being made. Complete only if you have never completed this form in the past or if you have been asked to complete one.
For a Waiver of Benefits Form, and the Long Term Disability Information Request form

Complete the Waiver of Benefits Form if at any time you elect not to be covered under Waco's health insurance plan.
Complete the LTD Information Request if at any time you wish to be contacted to discuss purchasing Long Term Disability insurance by payroll deduction.
